I hereby warrant the truth of the above statements and I declare that I have witheld no information whatever which might tend in any way to increase the risk of the Insurers or influence the acceptance of the Proposal.

I agree that this proposal shall be the basis of the Contract between me and the Insurers. I declare that my occupation and personal details and driving record do not render me ineligible to hire. The Hirer shall be liable for the first £200.00 of any damage to the vehicle, whether interior or exterior, including theft, damage to windscreen, tyres and mirrors, howsoever caused. The Hirer agrees that there will be no other Driver other than those specifically named on this Rental Agreement. The Hirer will be responsible for payment of all fixed penalty fines incurred by the Hirer under the Transport Act 1982 and the Road Traffic /regulation Act 1984. A £15.00 administration charge will be incurred for each penalty charge received. The undersigned (The Hirer) agrees to rent from the above (the Company) the motor vehicle described in this form for the period herein set out, subject to these terms and conditions which I have read and understood and which are to form a binding agreement. I have read the terms and conditions and agree to be bound by them. I authorise the owner to process a credit card voucher for the cost of the hire as set out below.
